Getting Ready for Your First Visit

Discover all of the information you need for your first visit to Innovative Dermatology.

We will make every effort to accommodate your schedule. Please let us know if you will be traveling so that we can schedule your surgery appropriately.

What to Bring

• Completed patient information forms
• Name, address and phone number of your pharmacy
• Insurance card(s)
• Driver’s license
• Any previous medical records, especially test results

Arrive early to fill out paperwork

Please plan to arrive 10 minutes before your appointment. Please arrive 30 minutes ahead of your appointment if you need to fill out your paperwork at the office.
